    • Eligibility:

    ATTENTION: YOU MUST BE HIV POSITIVE TO BE ELIGIBLE FOR THIS PROGRAM.

    - income must be at or below 350% of the current Federal Poverty Level
    - patient must have less than $25,000 in assets, excluding a home and car
    - patient cannot eligible for Medicaid or have health insurance that covers pharmaceuticals
    - must be HIV+ and provide proof of HIV+ status
    - must provide a prescription from a Florida licensed doctor and a current (Within 4 months) viral load and/or CD4 count test result
