Effective August 1, 2006, proof of lawful presence in the United States per HB 06S-1023 is required for new and renewing clients who are 18 years of age or older in the Child Health Plan Plus (CHP+) program. Clients in the CHP+ prenatal program are included in the population impacted by the new requirements. The new requirements include submitting identification documents and an affidavit with the application. Photocopies of the documents shall be acceptable identification if the photocopies are notarized or attested to in writing by a county caseworker or medical assistance site worker.

Child Health Plan Plus (CHP+) is low-cost health insurance for Colorado's uninsured children and pregnant women. CHP+ is public health insurance for children and pregnant women who earn too much to qualify for Medicaid, but cannot afford private health insurance.

Program Description:

Colorado Drug Card is a free prescription assistance program available to everyone living in Colorado. The program was launched to help the uninsured and underinsured residents of Colorado gain access to significant savings on prescription medications at their local pharmacies.

The Colorado Drug Card is working with the Colorado Medical Society, Denver Metro Chamber, and Colorado Academy of Physician Assistants to expand card distribution sites throughout the state. No personal information is required to join the program and there are no enrollment forms, no eligibility requirements, and no restrictions of any kind. The Colorado Drug Card provides discounts on both brand and generic medication with an average savings around 30%, with some discounts as high as 80%. The card is accepted at Walgreens, Safeway, Albertsons Sav-on, Target, CVS/pharmacy, Kmart, Walmart, as well as over 68,000 participating pharmacies across the country. SThis Program is not insurance.

Colorado Medicaid is public health insurance for families, children, pregnant women, persons who are blind or persons with disabilities and the elderly for Colorado residents. Family Medicaid and CHP+ are public health insurance programs for families, children and pregnant women. When you apply, you are applying for both Family Medicaid and CHP+. The program you qualify for depends upon family size, income and the age of the applicant.
