Eligibility:
Patient must meet the company's eligibility criteria for assistance. Financial criteria is based upon the Federal Poverty Level (FPL) and, therefore, eligibility varies with household size and will be determined on a case-by-case basis.

If the patient calls, they should have the healthcare provider's fax number and phone number, as well as the contact name at the office before calling the Patient Assistance Program.
