Eligibility:
Patient meets the company's financial eligibility criteria for assistance. Financial criteria is based upon the Federal Poverty Level (FPL), and, therefore, eligibility varies with household size and will be determined on a case-by-case basis. Income documentation is required.

To apply the healthcare provider's office should call the Bayer toll-free number 1-866-575-5002 between 9 am and 5 pm ET. If the patient calls, they should have the healthcare provider's fax and phone number, as well as the contact name at the office before calling the Patient Assistance Program at 1-866-575-5002. Healthcare providers and patients need to complete application forms. Both patient and healthcare provider's information must be returned to the Bayer Patient Assistance Program. Completed applications will be reviewed and approved by the Bayer Patient Assistance Program. The healthcare provider's office will receive notification in writing of the eligibility determination. If eligible a three-month supply of medication will be received at the healthcare provider's office usually within a week to 10 days.
