Patients who are uninsured or underinsured are eligible to apply. Underinsured patients are those who are experiencing coverage denials or have maxed out benefits. Qualifications are based on financial need. Supply: 28-day cycle.

Who Can Apply: Physician's office should initiate application by calling 800-931-8691. Application will be faxed to the physician for completion by physician and patient. Completed application should be faxed back to 800-822-2496. Eligibility outcome will be determined within 2 business days. Required: Physician's signature; patient signature (faxed signature is accepted).
