Eligibility:
Applicant's household income must be at or below 200% of the Federal Poverty Level, adjusted for family size. Applicant must not have any prescription coverage and reside in the US, Puerto Rico, and US Virgin Islands. Hardship Exceptions: Lower income patients who have insurance coverage for prescription medicines may still be eligible for assistance through the Pfizer Connection to Care patient assistance program if they are experiencing significant financial or medical hardship. Please contact Connection to Care at 866-706-2400 for more information.
