Xubex Pharmaceutical Services is the first patient assistance program that offers over 250 generic versions of branded medication, not available through any other Patient Assistance Programs. All prescriptions must be written for the number of days requested. When you visit your physician, please indicate a 90, 180 or 360 day supply. There is no limit to the number of different medications you may order. There is no enrollment fee or monthly charges and the medications are provided to you FREE of charge. An administrative fee is charged for EACH prescription and will be used to cover the costs of: Storing and handling the medications, processing fees, pharmacist and educational materials. $20.00 - 90 day supply $40.00 - 180 day supply $80.00 - 360 day supply A second class of drugs that is offered for $30, $60 and $120. All medications dispensed are FDA approved and made in the USA and dispensed by a licensed pharmacist. Automatic refills are available with prior arrangement. Once your application has been received and approved, your medication(s) ship within 5 to 10 days. The medication(s) will be mailed directly to your home, doctor's office or any other location you request. You must have a physicians' prescription for EACH medication that you are requesting. EACH household member must complete a SEPARATE application. Applications must be renewed annually. A current pay stub, bank account statement or Social Security benefit payment statement is required to prove income level.
